Please, do not make the same mistake as me.

The item is consumed on use. If your neck has 0 sockets and you use the item with quality 3, you will only get one socket.

To give your neck 3 sockets in the cheapest way, add them to your neck in the following order:

Quality 1
Quality 2
Quality 3

Just crafted Girocollo del Tempo Passato for someone and they confirmed that Castone a Strati per Medaglione DOES affect it!

For those wondering about if it's worth grabbing Girocollo del Tempo Passato though, the neck is upgradable up to 418 when using Infusione Primordiale Concentrata in the infuse with power slot. If you get yours at 392, there is a system called re-crafting that allows you to "update" the neck with new materials that you ended up not getting, add things you missed, and increase the quality. Re-crafting DOES NOT use another Scintilla d'Ingegno or Caos Primordiale Esaurito but does now use a small amount of Prova dell'Artigiano and less crafting materials than initial craft. In this process you can also change the selected stats and embellishment meaning you can easily swap it if you change specs or got the wrong items. I can't say if it's the best option to grab, however what I can say is that I'm a greedy jewel crafter who wants your gold.

Let me clarify a VERY EXPENSIVE AND COMMON MISTAKE real quick with some facts:

The items that are able to add a socket to a necklace are determined by how many sockets IT ALREADY HAS.
If a necklace has no sockets, you can use a Castone a Strati per Medaglione, Castone a Strati per Medaglione or Castone a Strati per Medaglione to add the FIRST socket.
If a necklace already has one socket, you can use a Castone a Strati per Medaglione or Castone a Strati per Medaglione to add the SECOND socket, you cannot add a second or third socket to a necklace using Castone a Strati per Medaglione.
If a necklace already has two sockets, you can only use a Castone a Strati per Medaglione to add the THIRD AND FINAL socket.

So basically, you can use the cheapest of the three qualities for the first socket, and the cheapest between quality 2 and 3 for the second socket, but will have to get a quality 3 for the final socket.

Edit: Confirmation that the above information is in fact correct. After further testing and publicly posted data, we can now confidently confirm that the description IS in fact misleading, and the text description `The quality of this item determines up to how many sockets it can add.` does not mean a Castone a Strati per Medaglione can add up to 3 sockets, it merely means the 3rd socket can only be added by a Castone a Strati per Medaglione.

As of the moment, the below "Tiered Medallion Setting" for a gem socket cannot be used on Collare d'Ossidiana, however Blizzard is currently working on a fix.

For what it's worth, this works on blue items as well as purple (epic) ones.

I used one to add a socket to an ilvl 366 blue item - a necklace dropped by Morchok.

I have a green L346 amulet with two stones. Got the three stone upgrade but cant use it on a green item. First I've heard of this.

there is currently a bug where if you have a stack of 2 rank 3 settings and apply them from the stack to a neck piece with 2 slots already in it
(in order to get the third slot) - both of the settings in the stack are consumed - have had them restored the 3 times it has happened to me

Currently this cannot be used to add sockets to a ilevel 424 (upgraded from ilevel 402) Girocollo del Vincolasogni. It apparently works with Girocollo Soffuso, so this seems like it may be a bug.

If you are wondering how to add the sockets to your necklace, open your bags, bring up your character pane, right click on the socket in your bag, the necklace will be highlited, and the cursor will change to a glowing gauntlet. Click on the necklace.

Many of these comments make the same mistake. You don't use the Tier Medalion Settings in the order of cheapest to most expensive. You use them from lesser quality to higher quality (one bronze diamond first, two silver diamonds second, three gold diamonds third). Price does not necessarily reflect quality. For some reason, a two-diamond item is often cheaper at Auction House than a one-diamond item.

It would appear these cannot be used on timewarped neck pieces, even of Hero/470-ish quality.

FWIW, I am unable to add sockets to a Season 4 necklace which came from the Timewalking reward cache (ie, NOT from a TW instance). I get the message "The quality of your Tiered Medallion Setting is too low to add another socket to this item." For the record, the necklace generated with one socket and I'm trying to apply a tier 3 medallion as a second socket.
