You can get one by getting Renown Level 25 with Spedizione delle Scaglie di Drago
Unsure if you can get them from the other factions as i have not yet reached 25 on them yet.

You can get Primal Infusion from

Getting Max Renown (25/30) with the four factions (one for each faction for a total of 4 Primal Infusions)
This quest that requires you to defeat Raszageth on normal or higher difficulty
Combining 10 Primal Focus and 100 Primal Chaos. You get Primal Focus from killing heroic bosses, finishing M+ dungeons (+11 or higher) and I'm assuming also rated PvP activities.

If I recraft an item to 399 with a Primal Infusion, but I want to try again to get a higher ilvl, does it require an additional Primal Infusion?

You can get Infusione Primordiale by getting maximum renown with each of the four Dragonflight main factions for a total of four. It says it in the renown description.

Dragonscale Expedition: Renown Level 25 - A Heroic Reward: Speak to Catalogatrice Jakes in the Dragonscale Basecamp to receive a Infusione Primordiale.
Maruuk Centaur: Renown Level 25 - A Heroic Reward: You will receive a Infusione Primordiale from the Maruuk for your service after a visit with Khanam Matra Sarest in Maruukai.
Iskaara Tuskarr: Renown Level 30 - A Heroic Reward: Speak to Murik in Iskaara to receive a Infusione Primordiale.
Valdrakken Accord: Renown Level 30 - A Heroic Reward: Speak to Unatos in Valdrakken to receive a Infusione Primordiale.

This can be used as an optional reagent to increase a crafted item's ilvl to 395 or higher.

Is there anything to use these on that doesn't require a Spark of Ingenuity? I'm drowning in these things and Primal Chaos, but Sparks are so limited...

This may seem obvious, but if you can't find yours they auto-deposit to your Reagent Bank and don't automatically pull from the bank or anything, they need to be in your inventory to use for crafting.

Per 10.2 these are obsolete, given you can fill up any missing items from the Dream campaign with Shadowflame Sparks (that you still get from Loamm weekly sources) and save Ingenuity for skill ups and transmog. Primal Infusions have no flow over item (in comparison to Focus Primordiale), so you're safe to delete these (and turn over Foci in rep!).

No longer obtainable via M+ or Segrete delle Incarnazioni: furia della Mangiatempeste, but it is still obtainable from maxing the main Renowns. Of course, if you're dedicated to play a character in the end game it's pointless, but if you just want to leisurely gear and you have a lot of rep tokens available you can max out whatever Renowns you need and use the free Sparks of Ingenuity you get from Motore dell'Innovazione to craft gear unobtainable with Emblema d'Ombrofuoco del Draghetto Incantato, which would be a trinket and engineering wrist (and engineering helm for Engineers).
