This item comes from having the Nadja soulbound with the renown 10 conduit. It's an upgrade to Encomio Scartato, but lasting 2h instead of 1h, and they stack. You can buy em from most dredger ruby vendors with the purple highlight, such as Asso Lutsione and the guy in Waynecrypt.

Very minor bug but don't buy it while sitting down next to the vendor. It'll put it in your bags instead of applying the buff (says You need to be standing to do that), and you can't use the item to apply the buff, or sell it back for anything other than copper.
