This fish is related to part 2 of the secret fish achievement series, Pesci segreti e dove trovarli.
To start this you'll need to complete part 1.

The goggles:
With the Occhiali dei Pesci Segreti buff applied a small fish in a bubble will spawn somewhere within 40 yards of you. If you have sound on you'll hear bubbles when it spawns then 5-10 seconds later the fish will become visible at which point you'll be able to click it. The fish can be clicked while mounted, you do not need to be in water for it to spawn and it will spawn every 25-30 seconds.

Best way to do this is find a spot that is flat and with few surrounding objects (trees, rocks, etc) and just spin your camera around until you see the bubble.

The Fish!
Each time you click the puffer you'll be given a random fish, most of the time it will be the white "junk" fish for whichever area you're in as well as the occasional grey item, with the exception of Dalaran where you'll commonly get coins usually caught from the eventide fountains.

Catchrate is quite low for many of these so expect to spend a fair amount of time for this.

I've split the fish into different catergories.

The common:
These fish can be caught anywhere at any time of the day, save these lot for last as you'll probably get most if not all of them hunting the others.

4. Pesce Imitatore Invisibile

6. Pinnasalina Collezionabile
7. Ghiozzo Viaggiatore

If you're still missing these after you catch the others, the best places to catch these are on the floating islands, like the ones scattered around Outland Nagrand (45.9, 59.9). Stand on the edge of the island and the fish will spawn in an arc around your character, shown here in green (Dalaran island used for an example): https://i.imgur.com/vrIYGgB.jpg

The zone exclusive:
These fish can only be caught in specific zones and sometimes even in very specific areas within zones.
9. Merluzzo Sangue Marcio - Nazmir, Zul'Nazman.
10. Pesce Storico di Giada - Jade Forest.
11. Sogliola Tonante - Isle of Thunder.
12. Pesce Pagliaccio del Kirin Tor - Dalaran (Broken Isles/Northrend).
13. Sgombro Meccanizzato - Mechagon.
14. Delizia della Regina - Nazjatar.
15. Manapinna Antico - Suramar. Anywhere in the Suramar city harbour. (Credit to Roguezs).
16. Pesce Prigioniero - Tol Barad (PvP area). Anywhere in the zone, the bridge that connects the two islands is a great place for this one, just make sure you're on the PvP area side of it (check minimap).
17. Pesce di Fiordo Spinato - Howling Fjord.
18. Liscavile Morto - Argus, Krokuun and Antoran Wastes. (Credit to Padwarle and Neryssa).
19. Tortolliano da Acquario - Drustvar, under Anyport in the underwater Tortollan bar called 'The Drunk Tank'.
20. Platessa dell'Aria Rarefatta - Neverest Pinnacle, Kun-Lai Summit. (Credit to Qaccy and Pungyo).
21. Guardingo del Pozzo - Mount Hyjal, in the lake under Nordassil.
22. Pesciolino Annegato - Drowned Lands (46, 50) in Stormsong Valley. (Credit to Bobins666).
Note: With the exception of the Tank Deweller (Fish 19), these are confirmed locations and may not be the only location within the zones where they can be caught. For the Tortollan Tank Deweller, I was only able to catch it once I was in the very specific sub-zone (where the name changes above the minimap).

For those suffering from woeful luck with the rarer fish, like the Platessa dell'Aria Rarefatta, this was discovered by Asmundr.

To everyone who is having trouble looting some fish. You need to approach the places like Neverest and Nordrassil on foot without a flying mount (There is a small path just before the Neverest peak) Then the 100% dropchance Fishes don't bug out and are lootable once more (Thin Air Flounder and Well Lurker) Alternatively just use the goggles when standing at those places when they are off CD.

If you're still having issues, clearing your bags of all secret fish (vendor or delete them) may also fix it.

The time scheduled:
These fish can only be caught within certain times, while one is active, the other is not. Whichever fish is active at the time can be caught in any zone.
23. Pesceluna Elusivo - Night only. 9:30pm to 8am.
24. Assorbisole Dorato - Day only. 8am to 9:30pm.
Note: We're not sure on the specific day/night switch over time, the above is just based on the times we've been catching them. If you catch these outside of the posted times let me know. Make sure its server time, not your local time.

The dead:
These fish can only be caught while the player is dead. Once you're a ghost these fish can be caught in any zone. (Credit to FenixInfinito).
25. Fantasma Velato
26. Pescevitreo Occhiuto
27. Galleggiante Silenzioso
28. Salmone Spirituale
Best place to catch these on any floating island, like the one directly behind the southern bank of Broken Isles Dalaran. Suicide by flying up and dismounting. This zone has ghost flying and if you stand on the edge of the island the fish will spawn in an arc around your character, shown here in green: https://i.imgur.com/vrIYGgB.jpg

The buffed:
29. Pesce Specchio Verde - Requires the Dipinto di Verde buff from the Mechagon mechanocat painting station (63, 42). This can be applied by walking over the green puddle on the station or having another player throw a Vescica Piena di Vernice Verde at you. (Credit to Sishrot).

The sporadic:
30. Pinnascarto Rimosso - Can be caught in the "future" version of Mechagon that Cromie sends you to for the quest L'altro posto. Unfortunately this daily is only available sporadically. (Credit to Lacrit).

Just got this a little earlier. The above (extremely helpful) guide says "under Nordrassil" - the pond just south of the main area where you teleport in from Org or Stormwind will have the fish. Also, I thought I'd be clever by hovering just over the water and plunging down to grab globes. That didn't work so well; they'd tend to be hidden by roots and such. When I swam down beneath the roots, I got the Guardingo del Pozzo and another of the "common" secret fish right away. Didn't even need a potion, in fact, as I wasn't down there long enough. Not sure if it's a higher percent chance to find them down there or if I just got lucky, but it's certainly easier to spot the bubbles without the roots or the ripple effect from the air-to-water transition in the way.

After sitting at 60, 25 and going up and down for 2 days and hundreds of fish, I went down to the second set of roots and got it on the very first bubble there.

So, an important note: If you didn't finish the quest yet which requires you to help Magni in Mount Hyjal, do it first, because even if you didn't accept the quest, Nordrassil will be in a different phase, where the azerite elementals are rampaging, and not the "original" one where the npcs are standing around. I learned it in the hard way...

Ok, so I "fished" this for the 2 hours my breathing potion lasted for with no luck. I had many bugged fish that would either start jumping all the way to the bottom of the well or they were unclickable.
After putting in a ticket due to frustration I got a reply back saying to make sure to delete the tank dweller. I didn't have one in my bags having read the forums and was constantly deleting. Other than that it was thank you for your patience but good luck with that.
Here's what I realised later. My heart of azeroth was ready for the next stage and while I hadn't collected the quest yet to upgrade it it had phased me to Hyjal anyway. There was a beast in the water getting in the way. Long story short if there is a beast in the water you aint gonna get the fish. Do the quest come back after handing it all in. Got it in 7 clicks.

Get Well Luker underwater, in Nordrassil lake, 2nd bubble, don't do any quests b4, maybe Underlight Angler helps, but only what i did take portal from Orgrimmar and jump in lake.
P.S. and get 26/30 other rare fishes in bag

Undoubtedly the worst one of all for me. Too many bubbles in the water and I have horrible eyesight. This was miserable. Try to get down far enough that the regular bubbles in the water disappear.

As of 8/11/2019, this one doesn't vendor (listed as "The merchant doesn't want that item"), whereas the others that I've caught so far vendor for 10s.

If you have Canna da Pesca di Sottoluce and fully maxed out. Equip it and you'll have your fish form and underwater breathing. Caught it on the first bubble. Yes, you can loot this while in fish form. I did have three other Secret Fish in my bags at the time.

Thin Air Flounder, Well Lurker and Tortollan Tank Dweller are the only three fish that can't be sold to the vendor.

Spent probably an hour on land trying to get this right by the portal, think I got most of the "anywhere" fish before I gave up. Came back and tried underwater, beneath the vines, got it on the first one I was actually able to click on.

THE WORST of them all!

Urgh I detest underwater activities and if you're like me and have none of the underwater breathing things, it is far more annoying.
The basic principle to this fish is simply get a couple breathing potions for underwater, then swim down enough that you can't see the regular bubbles in the lake anymore.

Then just listen for the noise of the spawn and swim around to locate it.
Problem is sometimes, the noise doesn't happen so just keep swimming about.

Annoying fish.

OK... initially i literally wasted almost 2 hours trying to find the fish... and nothing, even thought i was picking up some other rares... So AFTER you complete Magni's quest that's nearby, AND hand it in back in Silithus, THEN there are a few bubbles right near the bottom of the lake! Was no need to even wait for the spawn... could have been done in minutes...

I was completing this on one of my alts so, my Il Cuore di Azeroth was not up to date, it had me phased in Hyjal. There was a beast in the water getting in the way. Long story short if there is a Leviatano d'Azerite in the water you won't get the fish. Complete the Heart of Azeroth quest for Magni Barbabronzea come back after handing it all in. I looted the Guardingo del Pozzo in 1 click.

If you're spending a lot of time in the bottom of the well and having trouble with spawns, *try alternating* between the bottom of the well and 100-200 yards up. The spawns tend to happen once you drift away in the opposite direction of where you want them to spawn.

If you have not already acquired it, get the Redini di Poseidus Subdued Seahorse. It will enable you to swim up and down WAY faster than without!

Shadowlands Patch 9.2 here. I don't think that this fish is obtainable for the moment. I now tried to get it in the lake. At first in the upper area above the first roots. After that I tried it between root set 1 and set 2. After that I swam down to the bottom of the lake. After that I tried it under the tree in the western grass area. After that I tried it in the eastern grass area. After that.... I quit. I spent more then 5 hours an approx. around 250 trys. I got several secret fishs that are obtainable without being in a specific zone or area multiple times. But the well lurker doesn't drop. At all. No way.

Dragonflight 10.0.5, just fly to the lake and start swimming at the bottom bubble will already be there got it on 1st try did not do anything else.

Perfect advice about the Well Lurker. I did have a scare. I went to the bottom with my Secret Goggles and initially, it was not there. I forgot I had bought a water-breathing potion because this was so deep. I went back up to the mailbox picked that up and started back. I had not even gone down a quarter of the way and it floated right up to me.

FYI, in order to unlock Una ferita recente, you need to have your Heart of Azeroth at level 55 (I think) and you have to complete the quest Ritorno al mare/Ritorno al mare.
