To see this guy you must complete a Blade's Edge quest called The Thunderspike (https://www.wowhead.com/cata/quest=10526), the beginning of which is from a dropped item in Bladespire Hold.

When you have done the quest "the thunderspike" that "rokgah bloodgrip" gives you, garm wolfbrohter will spawn next to her as a ghost.

This may have been obvious to everyone else, but I died twice before I figured out that Rexxar is THE tank. Let him get aggro, then go DPS. You tank, you die.
