been looking at this and the reg version for transmog cause it reminds me so of Final Fantasy Dragoon Armor
so in that theme I have been looking for a Polearm to match only thing I have found to come close is Lancia del Rancore

Finally found a belt that matches good with this set, while not 100% color correct, it's as close as it can be.

I added a comment to KarlRhodes' question about potential belts, which I'll reiterate here:
Grancintura delle Acque Vivide or Cintura a Piastre del Guaritore da Guerra From Mythic Siege of Orgrimmar (the yellow version from regular Siege also works). Ignore the screenshot, it is incorrect, look at the 3d model.
And expand on weapons and cloaks. (Remember to try an orange or yellow weapon enchant glow, not a fire enchant as the shoulders exude mist in those two colours, not flame. Hopefully, at some point DKs will get Runeforging Illusion options, in which case Lichbane works nicely).

My personal preference is Mantello del Guerrafondaio drop with the Maglio Infuocato della Distruzione 2h mace, in which I sub in a different helm- either Corona del Gran Condottiero or Copricapo della Gloria. Seen here:

If no one wants to mess with trying to find a suitable belt to match, like myself...

The Acherus Knight's Girdle is invisible so it is sorta the best "match" in a sense. In my opinion of course :3

Paladins and warriors have never been able to obtain the shoulder appearance from this set because there is no lookalike piece for the DK Tier 12. I suggest Spallacci Corazzati Titanici del Trionfo, which is obtainable by all plate classes from the Wintergrasp Quartermaster vendors for Contrassegno d'Onorex2. Warriors can also use Spallacci a Piastre del Gladiatore Implacabile, their Season 7 class PVP shoulders, for the same appearance.

The brown plate texture on the shoulders coordinates with the "under-armor" portion of the Elementium set, while the subtle teal glow in the seams ties together with the Elementium set's dominant colors. Some races may see (very) minor clipping between the Elementium helm and the innermost spikes on the shoulder piece.
