This is a set that drops primarily from Ordalia dei Campioni. It is mostly comprised of plate drops itemized for tanking.

Head: Eadric il Puro/Espiatrice Paletress, heroic
Shoulder: Espiatrice Paletress, heroic. (Alternate: Guardaspalle della Stella Dorata, Oracles revered)
Chest: Pettorale Corazzato Titanico del Trionfo, Wintergrasp vendors. (Alternate: Pettorale Corazzato del Concilio Solenne, Wyrmrest Accord revered, and Carapace d'Osso di Balena, The Kalu'ak honored.)
Wrist: Bracciali Titanici del Trionfo, Wintergrasp vendors.
Hands: Espiatrice Paletress, normal. (Alternate: Guanti Lunghi Fuorimisura, Frenzyheart revered)
Waist: Cavaliere Nero, normal. (Alternate: Cinturone Titanico del Trionfo, Wintergrasp.)
Legs: Cassa del Campione, normal

There are two recolors:

Silver/gold, from Ordalia dei Campioni and reputation healing plate drops.
Black/silver, from Ordalia dei Campioni and reputation dps plate drops.

Please note that the two belts listed in this set do not share the same model. Cinturone Titanico del Trionfo (sold by the Wintergrasp Quartermasters) is probably the belt you are looking for: it is the 'round' belt shown on the picture.
Cinturone del Cavaliere Pallido, dropped by the black knight (normal), has the same skin, but uses the standard flat waist model. You could use the model-viewer if you want to see how that one looks :)

EDIT: As of patch 5.0, both belts now share the same round model, as shown on the picture.

In case someone wants this set and they use a 1H Sword and Shield, Malizia is a good sword that matches the set the best from what I've found. Akmin-Kurai, Scudo del Dominio Is actually the best shield I could find for this set, a few other shields that go okay with the set are as follows, Scudo Reale di Lordaeron is an okay match-up with this set. Scudo Pesante del Demolitore it kind of matches the color scheme it's not the best, but hey, who doesn't like a face for a shield? Difesa della Concentrazione Assorta di Antonidas is an okay match, still not better than Akmin-Kurai, Scudo del Dominio but it's okay. Scudo del Colosseo it matches the color scheme, but the eyes don't go with the set at all. If you find any other shields, or swords that you think would also go with the set then feel free to add them to this list. Happy farming!

This looks good but in my opinion swapping the shoulders for Conqueror's Siegebreaker Shoulderplates make it incredible.

Bloodfist Greaves (Schinieri di Pugnorosso) go really well with this set, if you don't like the way the boots look like re-skinned feet.

You cannot use Guardaspalle della Stella Dorata and Guanti Lunghi Fuorimisura to transmog this set. You need to be revered with the respective faction to equip these items and cannot be revered with both of them at the same time.

Best bet, imo, is to be/stay revered with the Oracles for the shoulders and farm Trial of the Champion on normal for the gloves because Spallacci Corazzati di Regal Aurous only drop on 5H ToC.

Non-bootie options:

Schinieri della Salvezza del Gladiatore Furioso matches pretty well if you're a warrior or pally who likes that shape.
Schinieri di Pugnorosso isn't too bad.

Stivalacci di Saronite Insanguinati colors are off, but might be an option when boots don't display fully

Post 6.1 update: seems the actual matching boots were finally added as a drop in Tanaan, which of course was after this comment was made. The others might still be worth considering on races that don't show the whole boot when the larger plate size is too big, most notably male tauren and draenei.

This is a set that is similar to the armor of Pacificatore d'Argento. It's various pieces can mostly be obtained through Trial of the Champion as stated by perculia above.

Head: Elmo dello Sfidante Battuto from Eadric il Puro or Espiatrice Paletress in Heroic Mode or Elmo di Titanacciaio Chiodato from 440 Blacksmithing

Shoulders: Spallacci Corazzati di Regal Aurous from Espiatrice Paletress in Heroic Mode or Guardaspalle della Stella Dorata from Oracles - Revered

Chest: Pettorale Corazzato Titanico del Trionfo which costs 490 from vendors in Wintergrasp (only available when Wintergrasp is controlled by your faction), Pettorale Corazzato del Concilio Solenne which requires Revered with the Wyrmrest Accord or Carapace d'Osso di Balena, which requires Honored with the Kalu'ak (the neutral Tuskarr faction found mainly in the Borean Tundra at Kaskala).

Hands: Morsa della Pietà from Espiatrice Paletress in Normal Mode or Guanti Lunghi Fuorimisura from Frenzyheart Tribe - Revered. Note that this cannot be used with Guardaspalle della Stella Dorata as it is not possible to be Revered with both the Oracles and the Frenzyheart Tribe.

Wrist: Bracciali Titanici del Trionfo, which costs 280 from vendors in Wintergrasp (only available when Wintergrasp is controlled by your faction)

Waist: Cinturone del Cavaliere Pallido from Cavaliere Nero in Normal Mode or Cinturone Titanico del Trionfo, which costs 280 from vendors in Wintergrasp (only available when Wintergrasp is controlled by your faction).

Legs: Gambali Foderati della Fedeltà Abbandonata from Cassa del Campione in Normal Mode of Trial of the Champion (spawns after defeating the Grand Champions in Trial of the Champion) and also from Cassa dei Campioni in Normal Mode of Trial of the Crusader (spawns after defeating the Faction Champions in Trial of the Crusader).

Feet: Stivali della Penitenza Sincera from Eadric il Puro in Normal Mode of Trial of the Champion, or Calzari della Tossina Infusa from Honored with the Knights of the Ebon Blade.

Anyone getting this set who doesn't want to look like they are wearing plate-socks, get the Stivali con Tacco di Ferro from Tanaan Jungle in WoD, they match this set and look like proper plate boots, not plate socks.

Picchiatore di Beazel is another awesome weapon to use, especially if you're a Dwarf seeking the perfect hammer for this set. Beazel's Basher is one of my all time favorites and blends quite well imo
