It may be possible that Archmage Tervosh is not in the tower when you get there. If he's not in the tower, be patient. It means that someone else if finishing off the last stage of the quest. Shouldn't be but a few minutes that you will have to wait.

Well, all you have to do in this part in walk straight into the tower and talk to him, he wont appear on minimap before you are inside since you have to discover the tower first.

Once you enter the tower, you'll need to look for Archmage Tervosh. He can be found at either of these three locations (I found him in the middle of Theramore Isle): The tower you are at, at the next stage or in the city.

One of my characters is offered this quest but , if accepted, it disappears on log-out. However, getting it again, I was able to finish the chain.

Blizzard must have left some loose ends in their code.
