There are 2 ways to get there.
Both of them require you to use Grapple point. Need to have Lancia Rampino dei Forgiatuono

Way 1 is to start from the tower at ~ 45.9, 72.0 (I'm showing this way in the video)
Way 2 will be available after you complete related part of the questchain.
You will see a dragon near the flight point 45.0, 59.3 Cullen's Post.
He has 3 options, select the first one.

At the Flight Point Lorna's Watch, behind the Flightmaster is a dragon named Vethir (accessible after some questlines completed)

Speak to the dragon and have him take you to the top of the Galebroken Path (first choice of the three he offers)

After landing, grapple across the bridge and continue straight forward. You will see another grapple point on a rock, and one more beyond that which takes you directly to Fathnyr's Cave.

I just died trying to get this dragon, I'm ok with that, the problem is since I'm dead I can not grapple to the entrance of the cave to get to my body, I'm going to get rez sickness to leave. I've died another couple of times for different reasons, and I had to swim because I can't walk on water, not sure if that is intented, and of course I can't climb rocks, considering the terrain, that is quite annoying.

Because the original content is a response, I'd like to up it:

If you die in this cave and can't get up (because haha, no grappling when dead) there is a path that starts from the side of the nearest graveyard that takes you above the cave ledge, allowing you to drop down onto it.
