While the world quest exclamation marker is at 50,17, he is physically located further in the spire. Follow the hallway until 44.94, 50.19 and you will be within striking distance of him.

If the World Quest Difendere la guglia is up, you'll have to complete that first in order to see Khazaduum, otherwise you'll be phased.
Yay.

If you have Khazaduum and Difendere la guglia together, you must complete Difendere la guglia first. Otherwise you can't complete Khazaduum because killing Khazaduum won't work while Difendere la guglia is active.

Ya he despawned on me half way through killing it and never came back. So annoying.

Coordinates: 39.7, 43.8 - Krokuun, Nath'raxas Spire

If the quest Difendere la guglia is active, you must complete it first to see Khazaduum.
The best way to go to Khazaduum is to click on the navigation console on the "Vindikaar" and get teleported to the Destiny Point. Follow the way down and you can see the entrance to Nath'raxas Spire from a distance. Go in and then go straight ahead until you reach Khazaduum.

It doesn't matter anymore if Difendere la guglia is up. You change phase when you go into the cave.

This may not be relevant for all but
Khazaduum was available for me only after unlocking Nath'raxas Hold World Quests
The chain starts with Dawn of Justice, and finish with Lord of the Spire
120 Fire Mage, 455 ilevel, at the moment of completing the chain
There still some Legion content I can't solo because the boss charm me or more than a player is required but it was not the case with this chain
