The source of this pattern has yet to be determined. MMO-Champion noted the addition of this pattern in patch 4.3 here, on 12/4/2011, stating "World Drop?" in regards to its source. Though, that page is subject to updates.

I recently went hunting for this to no avail for months, opened a ticket and only got the response:

"Drops, droprates and locations aren't really something we can discuss in certainty since we don't have access to that information any more than you would (that would be cheating =P). It's possible the droprate is just really low so no one has found it yet, or if it was only mentioned in a pre-patch note it may not actually have made it in from PTR. I'm sorry I can't be more specific on that."

Let's pray it ends up next patch or comes with Mop...
