When you first enter the keep take a right and keep following the wall right until you run into a bridge, you can see some greenery follow it straight and you will hit the catacombs. double check the screenshots for further tips.

When approaching the main gate, BEFORE entering, go to the right and follow along the wall. You will find a big crack that you can enter through. Turn left and head over the bridge. When you enter the courtyard, you go to the RIGHT of the building, then follow around to the back right side where you will see the entrance to the tomb.

Yes. The entrance is at the end of the path past The Sanctum. You're looking for "catacombs" because that's what it says in the instructions, but it's "The Tomb." Down the stairs, defeat 3 or 4 villains. Directly across from the bottom of the stairs is a little alcove with a white coffin. Click the coffin, get the head and Bob's Your Uncle.
