EDIT: This comment was made right when the patch hit. It appears that the 4 keepers and Yogg-Saron can each drop a random tier token.

There has been some discussion whether this drops in normal ONLY. I can confirm it drops both in NORMAL and HARDMODE, with HM being your usual mog run (i.e. not frolicking in the corridor for 3 minutes). You don't have to worry. :)
