What causes the Broken Bell treasures to spawn?

Is it like any other chest that just randomly spawns? Is it tied behind some quest chain? Need to be Kyrian?

I believe this toy can be obtained from any of the bell treasures in Bastion. I was able to loot it from a "Skyward Bell" and a friend was able to get it from a "Broken Bell". I'm not sure how many bells there are, but if the treasure on your map contains the word "Bell" in it, chances are it can drop from it.

I wonder if there's some hidden interaction or even a secret relating to these items? Vespro dell'Armonia, Vespro della Lucidità, Vespro Rassicurante (I'm not sure if there's others)
