Dropped off Stygian Incinerator for me. Pretty sure it can drop off any Maw rare just like the other lore items.

just another anti-AFK thing, this item is totally RNG, can be dropped off any mob in Maw and Korthia

well, last item i was missing... finally got it! went Korthia as a kyrian and i was camping Popo the npc close to the city that starts Wild Wordlcracker rare event, which event actually spawns the Opportunistic Mutilator mobs (2 of them) as ive read on wowhead with the highest dropchance. i was realmhopping to grps that were related to the Maw and/or Korthia (under Quest and Custom grpfinder tabs), wasnt tracking it but i guess my attempts around 200-300 kills ( 2kills/event), i wasted way too much time in the maw in a hope of kind rng... camp Popo its confirmed now!
