I separated my informations for pulling the Sword and put them in the Sorrowbane comments.

Inside the Theater of Pain in Maldraxxus (not inside the dungeon) you'll find the Sword Sorrowbane stuck in the ground and Braccio di Oonar attached to it. Both of them are clickable objects so try to grab them! Too weak? Then obviously you'll need to buff your strength!

Easiest Acquisition:

Buy a Pergamena del Teletrasporto: Teatro del Dolore from Joby Monocolo for 10 Gold or keep your Pietra del Ritorno off cooldown and bind it to the Innkeeper close to Theater of Pain (maybe check the location of Braccio di Oonar inside the arena if you haven't seen it before)
Head to Glutharn's Decay at far east of Maldraxxus.
Click 4x Amanita Commestibiles, then instantly use your Pergamena del Teletrasporto: Teatro del Dolore or hearth back to Theater of Pain.
Hurry up (80% slow) to get to Braccio di Oonar within 2min (maybe you wanna use a Kit dell'Aliante dei Goblin on top of the stairs when using the Hearthstone).
Click Braccio di Oonar to receive Braccio di Oonar. Congrats!

Buffs:
There are several buffs and their combinations that affect the text messages for pulling the Arm:

Temprato dalla Battaglia (10min, stacking up to 3x)
Amanita Commestibile (2min, stacking up to 4x)
Forza Esplosiva (2min)
Pozione della Forza Inusuale (25sec)
Forza del Sangue (1min)

Sources of Buffs:

Looks like Temprato dalla Battaglia is only available when Qualche ostacolo lungo la via is active in Maldraxxus' "The Spearhead" area. You have to defend damaged artillery by defeating NPCs close to it (filling up a progress bar) and get this buff when a ballista's repair is finished. (The WQ is completed when 2 ballistas are repaired and I don't think it's possible to help on another repair, so not sure why it can stack up to 3x.)
Amanita Commestibiles are small mushroom objects you can click. You can have up to 4x stacks of this buff active but keep in mind that each stack also applies a 20% slow. The only ones I found are in Glutharn's Decay in the eastern part of Maldraxxus together with Ecila il Ghiro. This shroom area is also the location of the Rare Spawn Lepiota (source of Timido Melvin battle pet).
I had to bind my Hearthstone to the Innkeeper near Theater of Pain or use Pergamena del Teletrasporto: Teatro del Dolore to get to Sorrowbane within the 2min buff duration.
Forza Esplosiva is the effect of a clickable vial on a table in House of Plagues area in Maldraxxus. For combining buffs I don't think it's a valuable option. I am playing a Shaman and had to click the Redcaps, hearth back to Theater, fly to Plague Watch, click the vial and Astral Recall back to Theater. Even with the now doubled duration I can only think of Shaman, Vulpera (Campo/Ritorno al Campo) or Warlock teleports being able to move that much within 2min, so not a match for an item you can carry until you want to use it. And there's another problem... you don't know if someone else clicked the vial and triggered the ~5min respawn timer.
Pozione della Forza Inusuale is the effect of Pozione della Forza Inusuale that is sold for 1 Gold by Au'larrynar in Maldraxxus (outside of the eastern Theater of Pain walls). You'll need to do some quests close to his position to unlock the vendor option.
Forza del Sangue is the effect of Forza del Sangue. You can get them from Ta'tru in exchange for a specific amount of a Shadowlands crafting reagent (changing weekly) at the Night Market in Revendreth.

Bind you Pietra del Ritorno to Bettol Valorum Theater of Pain windrider in Maldraxxus
Get 4 buffs of the Amanita Commestibile
Eat mushrooms on the ground near the Deadly Dapperling rare
They will slow you down

Use Pietra del Ritorno
Get to the Sword of Oonar item in the south-east side of the open world Theater of Pain
Hint: use Kit dell'Aliante dei Goblin near the stairs to get down faster

buy Pozione della Forza Inusuale from Au'larrynar in Maldraxxus /way 53.64, 47.91 Au'larrynar. To unlock this vendor you need completes side quests around the area until Effetti collaterali.
buy Forza del Sangue from Ta'tru in Revendreth /way 51,1, 78,8 Ta'tru. The vendor changes the price on his items every week.
grab 4x stacks of the buff Amanita Commestibile. You get this buff after interact with red mushrooms on Glutharn's Decay around /way 76,4, 56,7

Oonar's Arm (and Sorrowbane) are *not* part of the Treasures of Maldraxxus achievement.

If you have like me the permanent issue of "not getting out of fight again" and no reset possibility (i hate you, maldraxxus, for making me walk so much), get a Scroll of Teleportation, set your Hearthstone at Theater and use it after the Worldquest (if you can't get out of fight). Otherwise you will have no chance of getting to the shrooms in time.

A fun side note: when two players have this as their pet and they're next to each other, they engage in a game of "Rocks Paper Scissors". The winner gets a glow effect and the loser explodes and disappears.

Did it 3 times with 4 buff of Redcap buff (you can't get a 5th) + the Potion of Unusual Strength before it worked.
Worked for my guildie the first time they did it with 4 and potion.
*sigh* You'd think it'd be consistent :/

is it a reference to Honour? or Owner (of the sword?)?

I just did it 3 times on 3 different toons, with only 4 Amanita Commestibile and using the Pergamena del Teletrasporto: Teatro del Dolore (which put me within several steps of the arm) as per Lazey's comment.

But with how easy this was, makes me wonder why its selling for 45k. Is the arm you click a rare spawn? is the arm limited to one per toon? So having to get another requires making and moving another toon?
