To get Testimonianza Ombrosa you need reach Eye of the Jailer Tier 3/5 on The Maw by completing World Quests and Killing Elite/Rares.
When you reach Eye of the Jailer Tier 3/5 you gain the debuff Preda.
When you are being hunted, an assassin will strike when you enter in combat.
These assassins has chance to drop Testimonianza Ombrosa.

Also I get Testimonianza Ombrosa from Nutrianime Giurafauce. I got it after 4 trys.

This item drops pretty much from every mob in The Maw, rares and elites have much higher drop chance but still quite low. I'm sure that this quest item is the one of the last ones most of us will need for Deciphering Death's Intentions. Just keep doing your daily stuff in The Maw and hopefully you won't have to grind for it.

Just dropped for me for killing a random Mawsworn Outrider located in the beastwarrens

Just here to say after reading a variety of related comments, it dropped for me on the FIRST Mawsworn blackguard kill today. I had been farming them for days and days , running circles in the Maw with no luck. Someone hinted that it is a chance only at the first kill a day so for about two weeks I nuked one a day and went to do other stuff. Today, first kill, it dropped. FINALLY done the meta!
