UPD: All Horrific Vision toys have been found. Dunderman1 from WoW Secret Finding got this one figured out.

There are 5 trees with rotten apples around the Cathedral and Canal areas of stormwind:
3 at /way 57 55
1 at /way 60 58
3 at /way 65 62
4 at /way 72 67
3 at /way 67 51

Apples are clickable items around each tree (can be hard to see). When clicked they will either bounce around like a ball (cause they're actually spiders) or spawn maggots. Kill the maggots, and bounce spiders multiple times. After clearing all the apples, just go kill Alleria and check her Corrupted Chest.

I got it on my 2 try, so not sure if it's a chance enabled by clicking apples, or just some click not registering on 1 try. It is possible to do it on 415 character with just the orb upgrade (depens on class slightly).

ok, i got the 100% drop method:

1. it does not matter whr the 7 trees are, and no need to click them all

2. just go to ANY apple which can be kick/click infinitely (which means it won't spawn maggots)

3. kick/click 1 time, the toy drop chance will increase 1% - 2%, if u think u are unlucky, just kick/click 100 times, go kill the final boss and the toy will 100% drop

4. for security, u'd better not mount up to the final boss after ur kick/click, just walk/run to kill her (mounting up may reset the kick/click info)

just be patient, kick/click 100 times will garuntee u a ''must drop''

i've alr helped many chinese players to get this toy in one run, because it works

Just spending a few minutes kicking apples around at the very start region until I kicked a spiderapple at least 100 times, worked for me.

If you use a mouseover focus macro, you can use the interact on mouseover keybind (keybinds -> targeting ->towards the bottom, "interact with mouseover") and just spam that keybind on the focus icon. Coupled with a mouseover highlight from the game options, this makes it very easy to find and kick the apple very rapidly (and in fact, there were times where I believe I kicked it multiple times at once, as the apple's jump changed direction mid-flight).

Kicked 1 apple around 100 times, when it turned into a spider and started running back, i kept kicking it so it couldn't.

I got it first run.

Kicked one of the "spider" apples 100 times, and the Rotten Apple toy was in my final chest.

The rotten apples are difficult to see because they blend in with the cobbles. They're dark gray in color (not red), sort of like a... ROTTEN apple.

The tree that I found that had the apple around it wasn't any different than any of the other trees (looked dark and burned), but I'm not saying there might not be a difference.

For those new to WoW, fuller instructions to those listed by @darkscare

To create the macro

Type /macro
New
Give it a name such as Focus Mouseover, choose an icon and click Okay
In the Macro commands type: /focus
Click Save
Optionally drag the icon down to your toolbar
Click Exit

To assign it to a key:

Select Game Menu (the ? in your micro toolbar, or press Esc key)
Key Bindings
Targeting
Scroll down to the end of the list
Click the Key 1 button next to Interact With Mouseover
Type the letter/key that you want to associate with it (making a note if you unbind it's current function - you may need to assign it back after)
Click Okay

When you find a rotten apple containing spiders, place your mouse over it and click the assigned key to kick it. Repeat 100 times.

You can do this in a group. We went in with two. We just kicked the apple back and forth. Its sooooo much faster and even fun and the stacks are up quickly for both.
