You get this by getting Farseer Ori to rank 20, in which he will give you the quest L'ultima eredità. Simply turn it back into him and you will receive the toy.

It transforms you into an Ankoan model, though I'm not sure if the general appearance/clothing is random. It has a 60m CD, lasts 10m, and does not break when you get into combat, or for druid transformations.

When you learn this toy, you also learn the Libro dei Liberti toy. If you have a Horde character, you can use it to turn into an Unshackled kelpfin goblin thing.

EDIT:
We now have confirmation that getting 1 of these toys ( Talismano Ankoan Intagliato / Libro dei Liberti ) from getting the relevant follower to lvl 20, also grants the other faction's toy too!

Does anyone know if obtaining this grants us the opposite faction's toy too?
The Mount reward seems to grant both, and pets however do not, but no mention of the toy yet :(

The toy gives you different models each time.
I'll try and upload all the models i come across hopefully the admins here will accept the screenshots.
