Sfera della Trasmutazione di Silas turns into one of 4 random items:

Fiala delle Cure Continue di Silas: a reusable Pozione Costiera di Cura.
Bottiglia del Travestimento di Silas: a costume item, similar to the Moneta dalle Mille Facce (except non-player models).
Pietra del Trasporto di Silas: summons an interactable eye, which allows you to teleport to any npc hub in your current zone.
Pozione della Prosperità di Silas: doubles(?) the proc chance of your Rank 3 BfA alchemy craftables for 10 minutes.

And finally you can loot cauldrons for random BfA potions and flasks.

Buff lasts for 10 minutes on a 1 minute cooldown.

That being said, here are the actions that will remove the transform buff:
-Everything.

No really, once transformed, pretty much any action will remove the buff altogether. Using item, casting spell, mounting, etc. Which is kind of lame.

However, This item is also on a really short cooldown. So far the possible transforms I've seen are:
Brown bear.
Gorilla.
Ethereum void people.
Tentacle faced undead.
Turtle man.
Snake man.

Doing virtually anything removes the transformation. Trying to share the appearance by using a Reflecting Prism yourself removes the appearance. But another player can use the Prism to copy it from you. Once the appearance is copied it follows the rules of the Prism and so will remain through combat.

My findings also found out the following:

-Stealth before use does make the model stealth.
-Any action outside of running and jumping and talking to npc's will drop disguise.
-Gaining shrine buffs (for Zandalari Racial) will also drop disguise.
-Some of the same races have different outfits.
-You can still do emotes such as /dance
-Can eat and drink while transformed.

I am aware there are many other transformations, but I was not lucky to get them.

For my druid brothers and sisters, unfortunately any sort of shifting will also make the disguise fall off. Just another action to add to the 'everything breaks the transformation' but at least the item has a very short 1 minute cooldown. Would very much like for it to become similar to the Moneta dalle Mille Facce where shifting doesnt break the disguise or at least be able to cast or mount without it being taken off.

If you get the void ethereal look use a lightfoot potion while running for abit of a laugh! You look ridiculous for about 3 seconds

Terrible item, you are bound to not do anything while under it's effect. Most likely because they didn't want to bother creating new animations or something.
