This may scale to your level when it is completed instead of item level listed. My character was level 115 when completed quest and opened box to reveal it as a blue item item lvl 258 and grants 225 to primary stat for 12sec.

This box contains Intaglio di Maokka, which scales with your level until it is looted from the box itself.

I had this box in my bags for a few levels without realizing it contained anything, and when I looked at the contents, the trinket and its stats were scaled to match my current level. I closed the box, repeated the process as I leveled up, and noticed the same scaling results. The trinket will continue to scale as long as it is not looted from the box.

However, it appears that the quality of the trinket is decided when the box is first rewarded, as my trinket has never changed from rare quality.
