This trinket has an enormous absorb on-use that will root you and stop you from using abilities for three seconds. The trinket use could be shortened as "cannot be killed for three seconds" in most scenarios so it is like a cheat death every two minutes.

This trinket is much better than it appears, particularly for Guardian Druids.
The damage portion of the trinket benefits from versatility and tank artifact damage done modifiers.
The shield portion of the trinket benefits from versatility's healing done modifier... and a guardian druid's mastery, which increases healing/shielding taken.

Proof:
Trinket says it does 18.4m shielding on use. I have 14.84% versatility (healing) and 19.44% mastery (as a guardian druid).
When used, the scrolling combat text shows I get 25.24m shielding.
18.4 * 1.1484 * 1.1944 = 25.23m shields, which is close enough since the tooltip doesn't show the full shielding value.

To be fair, this isn't quite as broken as Frammento Purificato della Terza Luna was, since it only lasts 3 seconds instead of 20 seconds, but you can still use the Titanguard to basically ignore one mechanic every two minutes.

A few facts to remember with this trinket!

It can crit
You can hit a mob with more then one wave (Anything more then 2 waves is tricky)
It's fire damage, this means Fiery Brand for a Demon hunter will boost the damage.
Shield part is effected by versatility. (See Etamalgren's comment)
Not interrupted by pressing escape

This trinket SCALES with your level it seems. At 120 it is still shielding me for 118,050 damage. Considering my maximum health is 148k, that's still very impressive.

Edit: 10 days later - Well Blizzard ruined the fun - now only shields for 17,565
