Blight Boar is a mini-game at the Darkmoon Faire.
The concert event starts every hour on the half hour.

The concert takes place in an area with lots of Undead NPCs called the Cauldron of Rock. The Cauldron of Rock has a spooky stage and is surrounded by gravestones.
Every half hour, the Undead NPCs run into the area to catch a show! Producer Jay McGuire introduces Blight Boar and the NPCs all start dancing.
Periodically, a rare elite will spawn which you must defeat. This is a boss, the Cavaliere del Metal, who hits very hard!

How To Win:

During the performance, in order to gain points as a fan, you must protect the band.
You score points by defeating the Cavaliere del Metal and the Ghouls who spawn from the graves around the band and walk slowly towards the stage.
Stage diving also gives you points as a fan!
You will go from Fan Occasionale to Fan Serio to Fan Appassionato and finally to Fan Numero Uno as indicated by a debuff on your character.

Rewards:

New Cage Helms: Gabbia da Testa Inanellata, Gabbia da Testa Imbottita, Gabbia da Testa di Cuoio, Gabbia da Testa d'Acciaio. The use effect for the Guitar, Note di Morte Riecheggianti, transforms your character into Cage Head who is wearing a similar helm.
Microfono dei Cinghiali Appestati: Channel an epic Blight Boar performance. (30 Min Cooldown). Also transforms you into band member Devlynn Styx.
Necromedes il Riecheggiatore della Morte is an epic transmoggable guitar.

Achievements:

Ehi, sei una vera rockstar! Participate in the Blight Boar concert.
Pogo!!! Don't allow any ghouls to reach Delvin Styx during the Blight Boar concert.
These ghouls spawn from the graves around the stage.
Un'esecuzione perfetta Participate in a perfect performance of the Blight Boar concert.
This includes not allowing any ghouls to reach the stage and gaining a perfect score.
Perform a stage dive during the Blight Board concert.
To earn this achivement, jump off the stage through the fiery hoops!
Il tour del millennio Have each of the band's roles assigned to you after participating in the Blight Boar concert.

I think his rare loot is on a weekly/monthly timer.

You can enter this line into the game and if it says "true" it means the game flagged you as having already killed him and he will no longer show up as rare or have a chance at the rare loot.

I would like to confirm that you have a chance for the "special" loot (helmets, toy, 2h mace) only once a month, but for every single toon. The Death Metal Knight shows as rare-elite if not killed (and returns quest complete macro "false"), only as elite (and "true") if killed already this week.

I also want to confirm that toy and even helmet will drop on low level characters. I just got both the toy and the cloth helmet (item level 183!) on a lv79 mage. The 2h mace should follow the same rules (character level doesn't matter, but character must be able to equip it) to drop, i suspect, but I haven't gotten it so far, so no way to be sure. Also I'm not sure if the toy will drop below lv60, since you need to be lv60 to learn/use it.
So, go for it with every toon you got (at least lv60 and above), if you want that stuff. :)

Couldn't find a comment on what this toy actually does, so here you go:
Your character performs the first couple seconds of the song, and then stops.
Pretty underwhelming.

Q. So just where is the Blight Boar concert?
Can't see any concert goers. And you can’t hear the band from the main road, so the music won’t draw you in. If there are banners, didn't notice them.

A. Check the large map:
There are 2 paths to the venue, one near each end of the main drag. The paths look like the sides of an arrowhead. They join at a cave icon (Cauldron Rock).
Blue torches line the paths. The narrow banners for the Blight Boar band at the venue show a long green stripe topped by a skull. (No boar's head.)

*Don’t bother going to the waterfront - the concert isn't in the cave of the Coniglio di Lunacupa, it’s not on the floating wood platform occupied by Horde citizens and it's not up the hills.

Performances: According to an undead greeter, the concerts are every hour on the half hour. Be early or it might be over.

Toy still drops in Shadowlands 9.2.5. It states 14% drop chance however it took me almost 30 attempts. Remember for any chance for this toy to trop you can only kill once per month per toon. So I came in with 9 different alts over 4 months. I was loosing faith but finally dropped today
