Tome of Town Portal was an item that you would ideally purchase early in Diablo 2 to hold all your Scrolls of Town Portal. The original of course held 20 and not just 10. It starts off with very few uses, but can be refilled by finding or buying more scrolls.

One of the reasons this item was so iconic (other than being the Diablo version of a Hearth Stone) was that the Secret Cow Level (which does not exist) can only be reached in Diablo II by beating a difficulty of the game for the first time and then combining the Tome of Town Portal with Wirt's Leg (which has cameo'd multiple times in the Warcraft series) inside the Horadric Cube. Interestingly the Horadric Cube is a bag as well.

This item is combined with 10 Pergamena del Portale Cittadino to create Tomo del Portale Cittadino

Re Bovino is found by finding a Goblin Predone around the world (I found one just outside of Thunder Totem), killing it and venturing through the portal that is left by his corpse!

When you kill the king you get various charms and Pergamena del Portale Cittadino that are usable during the Diablo anniversary and also get the toy Chitarra a Dodici Corde which gives you the achievement 20° anniversario di Diablo

Just looted on Re Bovino. It's part of the event 20° anniversario di Diablo. You first need to find a Goblin Predone, a portal appears, enabling to access to the non-existing cows level !

This can also be looted from the Treasure Goblins. If you accidently clicked the portal or you forgot to loot, you can remove the "cow level" buff and you will be teleported back(unsure how this works with Treasure Goblins found at the end of Dungeons).

It seems that you cannot loot the Blank Diabolic Tome once you have already learn the toy (even with a toon), I got the toy with my main and I have killed several gobs (21 scrolls) and still no Tome so either I have bad luck or learning the toy prevent your char from looting the Tome

To be clear, you only need to collect 1 Tomo Diabolico Bianco which you combine with 10 Pergamena del Portale Cittadino to make the toy, Tomo del Portale Cittadino. I did not read the Wowhead guide's directions properly originally.

I got my Tomo Diabolico Bianco drop from Re Bovino. I farmed many Goblin Predone but I only received Pergamena del Portale Cittadino. I hope this helps!

If you plan on grabbing a second item of this, DO NOT CREATE THE TOY untill you get another in your PM mail, since they are unique and you won't have 2+ in inventory, then create the toy and grab the other.

Uh, I believe I just created the toy with 9 Scroll of Town Portal. Was farming the goblins in Dalaran Sewers and decided to just try and use the Blank Diabolic Tome after we missed killing two goblins in a row.
