This item gives you a quest Ritorno a Karazhan: La torre del potere. It is weekly item so if you did Karazhan before start a mission it will disappear.

The Head of Thar'zul is an item that starts a quest to enter Karazhan. The head is received as a reward from the Order Hall mission What Lurks Beneath the Tower?. The bonus roll is a Tribute of the Broken Isles. The quest from the Head involves going into Mythic Karazhan and killing Viz'aduum the Watcher. After turning the quest in at your Order Hall, you will receive a Glittering Pack that will give you a random piece of loot from Karazhan.

This item should be in your bags when turning in the quest The Tower of Power, otherwise it is shown as completed but button to turn in is inactive.

If it's like any of the other quests, this will take all duplicates from your bags when you turn it in. I've had it happen to the arena one, had 5, completed it and when I turned in, all of them were gone.
