We have two confirmed drops of this head from the eredar on the Highmountain Burial Grounds area (one from and another from Guardia dell'Ombra Eredar).

People have been farming this two for a while now, with no new drops, so it either has an extremely low drop rate, or it has a some other prerequisite ~~(current theory is that it's tied to faction rep with highmountain, both that got it were exalted with them)~~.

Update: A third head dropped on the same zone as the second one, it seems to just be a progressive grind after you meet the prerequisites of the first one (which are still unknown)

Final update: There are now several people (myself included) that have gotten the skin. After you get this first head, just keep farming eredar and you'll get every head (including the hidden skin) eventually.

I got all my heads in aszuna, farming . The first head dropped 8 mins in and total time for the skin was 1h32m, but there have been reports of taking up to 4h total.

As for the prerequisites, it's definitely rep gated, but we're still not sure of the exact reps. It could either be
Exalted Valarjar
Exalted Highmountain + exalted Valarjar
2 exalted non-Nightfallen broken isles reputations

Blizzard either changed or outright removed the prerequisites this tuesday. Just go and farm it, it will likely drop within your first hour of farming.

Rip everyone who grinded rep just to get this skin (aka me).

I got mine today droped in Arcway Mythic from the Eredar near General Xakal. I recently got my Demonology Artifact upgraded to Ilvl 871 so it might be that Ilvl of Artifact is pre-requirement to get it.

Got mine today from an Eredar Supplicant in Faronaar, Azsuna.

I was actually farming the book for the affliction hidden skin and was very surprised to see this drop, if you're going to be farming for the demo skin you may as well farm here, Killing two birds with one stone so to speak.

Even though the skull floating by your shoulder is always grinning he seems sad, maybe its the lack of expressions her can facilitate with his current appearance! Kill some Eredar to find him a new form to inhabit! The Damaged Eredar Head is your first hint as to what his after, you keep holding up head after head to him and every time you find something better than the last one (the other heads listed in the hidden appearance guide) you have a better idea of what he wants, ultimately resulting in your hidden appearance.

So what you actually need to do is kill loads of Eredar. The spot where I farmed this with a group of fellow warlocks I found in the custom group finder is the floating Legion spaceship thingy hanging over West Faronaar. The entrance is almost directly below it at 26,49 (Aszuna) and the Eredar Navigators that spawn will spawn about as fast as you can kill them even with a group of 5 farming warlocks. Ultimately this took one of the groupmates 3 hours to farm, I am currently at the 2nd after about an hour of farming.

Once you've farmed one of these, it's already tracked! Feel free to vendor them and keep on farming until you get the last one.

The Eredar (I guess that's what these things are) on the Broken Shore more specifically the Broken Valley South of where the Spider nest area is dropped both this and the Deformed Head for me today while I was farming for legendary quest items. I killed 384 demons 2 heads dropped, to give you a rough estimate.

These heads also drop from the eredar on Argus (I got two from the eredar in the Petrified Forest in Krokuun). Might make for easier farming since eredar are *everywhere.*

Had this drop and the Affliction hidden skin book almost within 5 minutes of each other coming to Argus. Blizzard did announce they upped the chance to find hidden skins for those who were still on the hunt.

Took me less then 10 mins to get them all - joined a group of 4 in premade LF under custom, who were farming intact demon eyes at the pit of pain ( a very popular spot for the intact eye grind and where i did mine earlier in 7.3
