Apparently (according to other players on this site) this item is given to Resto Druids that have their artifact equipped. Applebough will notice it and throw you an apple sometimes.

To anyone trying to get as many as you can (I'll take free water any day of the week), the only way I've gotten another is by logging out and back in. Applebough didn't toss me another after leaving Dalaran and coming back. I haven't tested getting one and then trying again some time later in the same session to see if there's a longish cooldown (that's reset upon logout). Will try sometime and post the results.

If I'm not mistaken this is the best (maximum health/mana restored) non-buff food in the game right now, making this a nice "every now and then" savings of the ~3 gold you'd spend on water (or bread). It'd be nice if he threw a stack at a time, but it's free so whatever.
