Dropped from a horde NPC during a PVP world quest at Darkfollow's Spire in Val'sharah, even though the quest location is in Highmountain.

Edit: Got another, this time from Cordana's Apex. Starting to think you can get it from any horde NPC at those towers.

He's behind the wall behind Brogrul the Mighty. To get to him you'll have to kill Brogrul, then carefully inch your way around the corner of the wall.

You can farm this out by farming the kobalds to the right of Skyhorn.

Just dropped for me of a drogbar during Assault on Thunder Totem world quest, tried farming kobolds for an hour over several days without success.

Got it really fast from Bluewax Graverobbers outside the cave. Stand inbetween the big group and remove your Friends for Life buff. With serverhopping you'll get a pretty high kill rate.

Just got it right now farming Gilnean Mauler for less than 10 minutes, I recommend that right away! After farming Kobolds for 1+ Hour and only getting Fevered Letter instead..

Unfortunately this one dropped for my alt rather than my main, which is focusing on getting the achievement. Anyway, this dropped off the raptors (surprisingly) from the WQ Snowfeather Swarm! in Highmountain.

I may try this again on my main to see if it drops.

Update 1: I did it with my main. Crumpled Note dropped instead, which I've already done it.
Update 2: Got this quest within roughly 10 - 15 minutes after i abandon the Crumpled Note quest.

Although the drop list of Bluewax Graverobbers (on Wowhead) does not include this item, I got this from them as well.

We discovered that one of the best spots to farm for this item (along with Lettera Febbrile and Nota Accartocciata) is in Snowblind Mesa (53,62). The Mightstone drogbar respawn instantly (it's a neverending stream, they just keep coming). You just need to tag them before the NPCs get a chance to kill them - not a problem if you are ranged or farming with a group.

Crawshuk the Hungry's own page give the drop rate of 0.01% for this item. Calls into question the entire drop rate database, in my opinion — the whole site. Alas.

I collected on kobolds for 6 hours. Finally succeeded. I confirm that it dropped out with Bluewax Cavewhisker.
