Just tested it: you can skip Nhallish and Bonemaw, Ner'zhul will still spawn.

Droprate is probably around 10%.

This can be obtained on any mode, including normal mode, which means you can run it over and over until it drops if you wish.

Obtained it on my 4th run on normal difficulty.

I'm on run 19 on normal mode with no drop. I'm either unlucky or this droprate is 5% or lower.

Does have low drop rate. Ran 20 times on normal LFG until it finally dropped. Good luck!

Got the Shadowmoon Illusion on the 6th run, all normal. Sometimes he drops loot, sometimes he doesn't. That just means he dropped loot you weren't eligible for. All 6 times I skipped the worm and the purple ghost bosses. Doesn't matter if they aggro. Just ignore everything and run through the instance top speed. 110's can take it.

As of today, patch 7.2 it took me 13 runs on Normal to get the Illusion:Mark of Shadowmoon

As a 110 sPriest, I ran all the way to last boss w/out taking a hint of damage due to the bubble from Prydaz, Xavaric's Magnum Opus as well as the Power Word: Shield that i popped on CD to get my speed boost. Then, i quick pop of Kil'jaeden's Burning Wish make short work of everything, although all you need to do is kill the 2 foolish Void guards to get access to Ner'zhul behind the wall.

In the end, no matter what class, spec you chose to farm this on at 110, it seems an average player will see this drop sometime between your 1st and 20th run unless The Void Lords themselves are against you owning this beauty of the Void.

First run on mythic: no drop. Second run on heroic: no drop. Third run on normal: drop.

If you click it to use and you already have it it will inform you now ,unlike in the past it would use it and do nothing. I just confirmed this today. June 7 ,2017.

Fun trick that works since they nerfed the two boss skip trick.

1.) Clear the dungeon on normal as usual.
2.) Once you kill Ner'zhul click his portal and run out of the dungeon
3.) DO NOT RESET - Run back in - you'll be spawned in Bonemaw's stage
4.) Log off for approximately 20+ minutes (I'd tank a dungeon on my other toon or watch Netflix.)
5.) Log in and you'll spawn in front of Bonemaw, kill him, run through one group of adds, kill the two Void Spawns and kill Ner'zhul

Rinse and repeat steps 2 through 5 until the Illusion drops.
