I got one from For the Cause quest, Rogue Class Hall.
The reason I'm writing this is because the quest page, at the moment, do not mention this. In fact, the only source seems the emissary boxes.
Seeing Alfa's comment, it looks like this item somehow escaped the data radar.

NOTE: While you are prevented from using this to upgrade a Champion that is already at max iLvl (850 at the time of writing this), it will be consumed even if your follower is less than 15 iLvls from max, and thus you will not get its full benefit.

Stated another way: if your Champion is at iLvl 845 or 840, you will only get 5 or 10 iLvls of upgrade out of the 15 that this item is capable of, so you might consider saving it to upgrade a different Champion.

Can anyone confirm that this only gave +10 item level ?

This was applied to a 780lvl follower. Only advanced to 780.

Is there any reason to hoard these? Will we gain new champions throughout the expansion, will their item level increase from 850? or should I just get rid of them?
