For those curious how this item works:

Picture the 15 other "Keg" items in the game: you place it and then you can right-click it to get drunk.

Blizz introduced this item for one key purpose. You see, they used to have those other keg items clutter your bags, but then they mistakenly introduced the toybox so that you wouldn't have to clutter your bags with them.

This item was introduced to fix that mistake. It clutters your bags once again. We can only hope that they decide to change all of the other kegs back into non-toys again.

EDIT: It's now a toy again. Here's to hoping they fix their mistake and add in another non-Toy keg in the near future.

The really special thing about this item, is that it cannot be DE'd or sold. It is truly special. I know this because I tried both on this SPECIAL. It should be the item left by the ghost dog ... it would be more appropriate.

For Alliance:
To obtain this toy you have to unlock your outpost by completing quests for arrakoa outcasts untill your scout directs you to Southport. After that pick up the quest Ottenere l'amicizia degli autoctoni and complete it's short chain. Upor returning to Southport pick up Tentato omicidio.
After you complete the last quest in chain wait for Kolrigg Birrafredda to walk into the inn and he will offer the quest that rewards the toy shortly.

The Barile Misterioso can only be obtained if you have built the Hearthfire Tavern (Brewery) instead of the Smuggler's Den as your Spires of Arak Outpost. If you have already selected the Smuggler's Den, you must pay 5000g to switch outposts (You can get 1000g back later, as I've noted at the end of this comment). This can be done by speaking to Coordinatore Gornek (H)/Tenente Willem (A), who will give you the option to pay the fee to switch.

After building the Brewery Outpost, you must complete the following quest line:

After the quest Non c'è tempo da perdere/Non c'è tempo da perdere has been completed, you can obtain the quest Non nella tua armata (H)/Non nella tua Guarnigione (A), which will grant the Barile Misterioso toy as a reward. In addition, you will get a follower for your garrison - Hulda Lamaombra / Guardaboschi Oscura Velonara (H)/Hulda Lamaombra / Guardaboschi Oscura Velonara (A). This follower is also available from Nalya Suona Corni (H)/Deluwin Soffiacampi (A) in your garrison for 5000g if you have built a Smuggler's Den and do not intend to change it.

NOTE: Completing Non c'è tempo da perdere (H)/Non c'è tempo da perdere (A) also grants Note sulla Costruzione degli Edifici dell'Avamposto as a reward. This can be used to purchase blueprints from Torgg Truffalotti (Warspear)/Kinkade Jakobs (Stormshield). You can get blueprints you don't already have instead of purchasing them with gold.
However, if you already have all the blueprints, purchase any of the following blueprints to vendor for 1000g:

Barile Misterioso is one of seven unlimited-use drinking items available in game.
Barile Misterioso is rewarded from the quest: Non nella tua Guarnigione (A) / Non nella tua armata (H) in Guglie di Arakk.

Note: to be eligible for the quest, you must build the Stoktron Brewery (A) / Hearthfire Tavern (H) in the Guglie di Arakk outpost.

The other items are:

Ultimo Boccale di Lao Chin - a quest reward from Barili rotolanti! in Massiccio del Kun-Lai.
Birra di Jin Pinta Calda - a quest reward from Rintanati in Massiccio del Kun-Lai.
Kit da Birra degli Ogre - a guaranteed drop from Pestatore Kreego, a rare mob found in Gorgrond (location: 38.2, 66.2).
Pony Portabarili della Festa della Birra - sold by Brewfest vendors for 200 Gettone della Festa della Birra during Brewfest.
Barile della Festa della Birra - sold by Brewfest vendors for 100 Gettone della Festa della Birra during Brewfest.
Zaino Birra dei Pandaren - sold by Brewfest vendors for 100 Gettone della Festa della Birra during Brewfest.

There are also six unlimited-use food items available in game:

Bottino della Raccolta (creates Pane del Raccolto) - mailed to players after completing Onore all'eroe (A) / Onore all'eroe (H) during the Harvest Festival.
Foglia di Sottospora Perpetua (creates Baccello di Sottospora) - item is now unobtainable.
Avanzi Perpetui - contained in Beni da Nomi which is a quest reward from Un segno di ringraziamento.
Bocciolo dell'Abbondanza di Ba'ruun - a guaranteed drop from Ba'ruun, a rare mob found in Valle di Torvaluna (location: 53, 16).
Pasto Delizioso di Talador - contained in Cestino della Merenda del Caposquadra, a treasure found in Talador (location: 57.39, 28.66).
Succo di Funghi Stigio Senza Fondo - contained in Scorta di Funghi dei Guerci Insanguinati, a treasure found in Giungla di Tanaan (cave entrance: 44.6, 77.5 / treasure location: 49.9, 76.8).

You can only get this toy if you have the Smugglers Outpost in Spires. If you chose the other building and already got the garrison follower and the toy "Bloodmane Charm", you can still get the "Mystery Keg". What you need to do is speak to Lieutenant Willem in Southport. He will give you the option to change your outpost building for 5000 gold. After peaking to him and choosing the Stockton Brewery building, Lieutenant Willem will have a new quest "Befriending the Locals". Before I switched buildings I already had all the quests in Spires complete so this was the only new quest available. The quest chain is as follows...
