To get Chiave dei Bastioni Superiori you need reach Eye of the Jailer Tier 3/5 on The Maw by completing World Quests and Killing Elite/Rares.
When you reach Eye of the Jailer Tier 3/5 you gain the debuff Preda.
When you are being hunted, an assassin will strike when you next enter combat.
This Assassin will drop the Chiave dei Bastioni Superiori (i got on my first kill). Note: the assassin drop another items, for example: & Testimonianza Ombrosa.
After get Chiave dei Bastioni Superiori just complete the quest (auto pop).

Worth noting the daily quests you get from Ve'nari each grant a full Jailer tier.
