I only had to kill five Disertore Maldraxxi before it dropped. It might have dropped from the fourth or fifth one, I killed them both and then looted so I'm not sure.

the mission vanished from my mission table and i cant get the quest item to drop.,
killed everything several times over

This dropped for me while doing the storyline quests, although my character reached level 60 at some point through the previous chapter. Looks like you do not need to have chosen a Covenant to get this quest to drop. I was farming ore nodes while doing the storyline, and one popped up nearby. Killed a couple of these mobs who were near the ore node and one of them dropped it.
