Though the above posts were similar to my first thoughts, I have to play the Devil's advocate.

Not every item/quest/NPC is a pop culture reference. It's entirely possible that the hatchet is called such because psychopathic murderers are often associated with performing their deeds with such tools. It is, in fact, how the above mentioned record company came up with their name in the first place.

this is almost definitely a reference to ICP... aside from it being called the Psychopathic (icp's record label) Hatchet, the picture itself is a *bloody* hatchet, which i haven't seen anywhere else, and the icp logo features a distinctly bloody hatchet.
they could have gone with any other image for the thumbnail.
either way, icp has been around for 20 years. they're not going anywhere. in fact, they've promised the release of another 5 albums.
