We really shouldn't be talking about Kael'thas here... but he didn't come back to life, he just never died. Priestess Delrissa tended to his wounds after we left. Quite simple.
I don't think Blizzard believes the Lich King really needs the helm. It's more of a symbol. The loading screen for WotLK is the Lich King without his helmet on.Therefore, I believe the animation is simply the Lich King grasping for the symbol of his power before he dies. To be more specific, in his dying moments, after he has been defeated, he is powerless. He reaches for the helmet in an effort to regain a sense of power and strength but cannot. He dies.
My guess is this.The LK is NOT dead.Ner'zhul was trapped inside the armor, trapped inside the throne. Telepathy seemed to be Ner'zhul's primary ability (as well as any necrotic abilities). Ideally, this would make one believe... of all the possible pieces of armor, the helm is were he was.When Arthas put on the helm, that's when they merged.When you think about it, the LK as we see him is nothing more then a dead vessel. It clearly has no vitals (not even undead ones...). Think Krang from TMNT.Ner'zhul absorbed Arthas. Mind and spirit. The body was, really a replacement for the throne it had been stuck in (and was loosing power).Now, really the LK is probably really weak. Because his containment for his powers (the body) is gone.LK dead... naw. Some stupid person is gonna put the helmet on. The LK will awaken and consume them.OR..The LK will be brought back, by his former master Sargeras.Really, there's a lot of weasel ways out of this. Especially when you consider the fact that... well, UNDEAD.
I now think that the video is a distraction from the real death scene of the Lich King.When you almost kill arthas, Tirion drops from the sky yelling "ASHBRINGER, HOOOOO!" before carving him in half.
Well since the question is being asked "Where is Frostmorne" allow my input. Aslong as The Lich King has the armour and Frostmourne, he is invulnerable against everything EXCEPT the ashbringer. I would say that Tirion attacks him, and the Ashbringer breaks Aswell as Frostmourne. The surge of power from both weapons kill each hero i.e. Unholy from frostmourne kills Tirion, Holy from Ashbringer severely injures Arthas and Ner'Zhul The Lich King. Now, he falls before the holy power, his helmet falls off. His power is weakened by the loss of Frostmourne and his helmet, with the holy light still coursing throughout his body = 1 Dead Maniac Being.Just my thoughts.
