This NPC only spawns if you have the dailies for that area and it has been confirmed.

I had the dailies for the area where Karanosh can be found and it was there today. But yesterday and a few days before when I did not have the dailies for that area, Karanosh was not to be seen within that area of the zone.

One of three bonus dailies for Klaxxi in Dread Wastes.

The Briny Muck / Zan'vess. If you happen to be questing in the area, swing by and kill Kypa'rak. He will drop Massive Kyparite Core, which starts Il nucleo di Kypa'rak.

The Clutches of Shek'zeer. When questing in this area, you will be able to spot Karanosh. Same idea here - kill her, loot Enormous Kunchong Mandibles, start Le fauci della matriarca.

Terrace of Gurthan / Forgotten Mire. Finally, if you get this quest hub, go kill Warlord Gurthan for his Ashes of Warlord Gurthan, which start Le ceneri del Signore della Guerra.
