Finally I got this after 10min farming from a Segugio d'Ossa Totem Vile after religiously doing the pvp quests in Highmountain for months with no success.

I started farming for this, as it is my last for the achievement. Yesterday I farmed for about 4 hours from Grey Bazaar, Cordana's Apex and Bloodhunt Highland. I have looted 3x Crumpled Letter. I completed the quest the first time, abandoned the 2nd time and Just looted the 3rd one from a Feltotem Bonehound. I decided to take a look at the loot table and noticed the bonehound doesn't have the Crumpled Letter on it's loot table. I wonder if this drop is bugged.

Finally got this after around 4-5 hours farm from a Gilnean Huntmaster in Cordana's Apex don't give up ppl! :D

This just dropped for me from a Stormheim worgen NPC in the pvp world quest towers so you don't need to farm this in Highmountain.

This drops from the Bluewax kobolds near Skyhorn, you don't have to go in the cave just farm the ones outside.

I got the crumpled note off the 4th Steelgaze Basilisk I killed in the cave at 54,75, right next to Frostshard.

I can confirm that the drop is not restricted to Highmountain. Got it today from the Undead PvP WQ mobs in Valsharah. Had 750 skill which is messed up since i can only abandon the quest because i don't have the skill learned.

Edit: Got it again - this time from a Sharptalon Dino in Stormheim .

just bare in mind that if you do this before you have 800 skill in first aid you wont be able to complete it, so don't waste your time farming this one until then.

After months and months of farming all of the recommended spots, this finally dropped off of a snowfeather, during the world quest Snowfeather Swarm in Highmountain!

May 25th
I went there (Torok's Bluff) to farm for the Nota Accartocciata while the WQ was active and Segugio d'Ossa Totem Vile were replace by Demone Totem Vile

May 26th
Went there this morning, no WQ, and Segugio d'Ossa Totem Vile were there. They seem to be on a 5 minutes respawn timer. I tried to kill other feltotem in the area to see if they share spawns with them, they don't.
Still no WQ active but it's now WQ reset time (11 am EST) and Segugio d'Ossa Totem Vile have been replaced by Demone Totem Vile.

May 27th
Demone Totem Vile before reset (11 am EST)
Reset (11 am EST) Segugio d'Ossa Totem Vile are back.
2 pm EST we are back to Demone Totem Vile
Will check back at 5 pm EST and report back here.
5 PM EST. Segugio d'Ossa Totem Vile up
8 PM EST. Still Segugio d'Ossa Totem Vile.
and the other days after with no success
I finally decided to go Cordana's Apex in Highmountain to continue farming for the Nota Accartocciata.
Because a lot of comments were pointing there.
It dropped after 30 minutes.

I just picked one up from the Vilewing Screecher harpies at Screeching Crag (46, 47). The world quest was *not* active, I just started killing them after picking up a chest, and it dropped.

Crumpled Note received from the Bluewax Kobolds in Highmountain. You can find the cave behind Skyhorn at /way 55.18, 44.31.

Tip: Take the skull off of your head. The skull makes them neutral. If you remove the skull, you can pull more than one at a time.

Working on pre-Battle for Azeroth achievements and stopped to do the "Water of Life" world quest for order resources. Crumpled Note dropped off the Drogbar at /way Highmountain 38, 60.2. Nice timing.
