While it's not stated in the item text (or in any other comment) you can see the spell seems to have a 10 min cd, if you hover its text with the mouse it will show the "Eternal Sacrifice" 's tooltip.

About its uses, they are far more than just a quick way out of a boss fight.. imagine running around orgrimmar (or stormwind) on a pvp server, taunting everyone while being completely immune for 3 minutes, just to heart out at the last second...if you find a paladin friend who will willingly lend you his/her help, that's it.
