This is the heroic T13 Shaman gearset. Two other colour variants of this set exists: the raid finder set in blue and the normal version set in yellow. The shoulders of all three sets are animated, in that occasionally a ghostly wolf head will emerge from each shoulder along with faint misty swirls.

Some matching items:
No weapons or off-hands/shields of the same tier match this set. However, a close colour match to the set would be this shield The Boreal Guard from Ulduar.

As nearly all transmogged weapons display the currently applied enchant, then finding a matching main hand can be tricky.

Some matching weapons that don't show enchants are:
Dagger - Darkmoon Dirk or Sorcerous Dagger
Axe - Amani Venom Axe or Bogreaver
Mace - no great matches, Mariel's Sorrow probably the closest

IMHO, some items matching this set:
Shield - Orgoglio dei Kor'kron (Horde-only, Alliance version of the same shield Scudo Guardia Vigilante might be used as well)
Mace - Serrafauci, Ultima Parola, Martello Robusto Crepato, Soglia della Sofferenza (Horde-only)
Dagger - Lama Incantata di Titanacciaio, Rathrak, la Mente Velenosa
Axe - No'Kaled, gli Elementi della Morte, Furia Sanguinaria (Horde-only, Alliance heroic version Sfregiatore Testa di Leone is OK as well)
